  • Sent at baseline, then at 1, 2, 3, and 6 months after initial visit
  • General Health Questionnaire
  • Keele STarT Back (at baseline only)
  • New episode of back pain or a new flare that began in the past 6 months
  • Consent to participate
  • Physical Examination indicates pain source is lumbar spine
  • Identify data entry errors
  • Educate team about entry process
  • Catch error patterns early
  • Insure that data is high-integrity - a requirement for quality based incentives

Enrollment options to fit your practice style

  • Clinic enrolls patient in the registry using a link requiring no log in and enters clinical information
  • Clinic faxes enrollment and clinical information to the Coordinating Center for entry
  • Epic-based providers can have their information abstracted from the EMR (access must be granted) - hopeful to add more EMR systems in the future
  • Patient surveys above are automatically pushed to patient via email or text once enrolled if not done on paper in the office and faxed to the Coordinating Center
  • Practice can opt to complete all initial surveys on paper and fax with enrollment and clinical information to the Coordinating Center for entry
